How does it pick the language?

From the caller's first words. If the caller switches mid-call, it switches too.

What if a caller asks for a language that is not live?

It says which languages it can speak today and that more are coming, then carries on in a live language.

Does a language cost extra?

No. Every live language is included in the per-provider price and the same $0.20 a minute.

When do the next languages go live?

Hindi and Hinglish are built and being tested on real calls. A practice that wants them early can ask on the contact page.
