  • Nerve conduction codes are chosen by the total number of studies on the day, and EMG is reported as the add-on (95885, 95886) when done with them.
  • Botulinum toxin for chronic migraine (64615) is bilateral by definition; do not add modifier 50. The drug (J0585) is separate.
  • Prolonged EEG codes (95700-95726) are reported per 24-hour period with the video and monitoring level documented.
